In Japan, due to the rapid progress of the low birth and aging, a large fiscal burden is placed on welfare expenses for the elderly from the viewpoint of social security. Since the super aged society and the long-lived society called “100 Years of Life” will become in the future, the direction of social security policies and welfare policy for the elderly is the key to reducing medical care-related expenses. As a social welfare policy for the elderly, construction of various nursing care facilities and dedicated housing for elderly who are collectively referred to as “nursing homes” has been promoted on a policy basis. However, the welfare philosophy is being shifted from “family care type” that family members supports the elderly, to “social care type” that society supports jointly the elderly. In other words, the welfare philosophy is changing from the “family (individual) responsibility” by traditional thought to the “social responsibility” by realistic thought. The background of this change in welfare philosophy is that as Japan becomes economically rich, modernization, nuclear familization and birth and aging become more advanced, living in a family unit (living together, supporting together) became difficult in reality, and family members were inevitably broken down by economic and social activities of family members.
In addition, there is also a reality that the scope of social security as a nation is stimulated by the philosophy of social security in Europe and citizens are seeking a policy change from passive social security to aggressive social security. In the future, the basic direction of social security policy that supports the life of the elderly is inevitable to change from “family” to “society”.
